Preparing for Your Initial Appointment

We want your visit with Lexington Surgical Associates to be as pleasant as possible.

After making your appointment, please obtain copies of any pertinent medical records or test results done by other physicians to bring with you, or have them sent to us. In many cases surgery cannot be considered until this past medical information is reviewed by the surgeon.

We will need to get copies of your insurance cards at the time of your visit if you would like for us to file for any surgical procedures or office visits. PLEASE BRING YOUR CARDS WITH YOU. We will be happy to file up to two insurance claims at no additional charge; however, we must charge a fee for completion of more than two health insurance or disability forms.

For office consults only, we request that you pay at the time of your visit unless we are members of your managed care program, in which case you will be required to pay only your “co-pay” amount. Please be familiar with the amount your particular plan requires you to pay for specialists, and be prepared to pay this at the time of your visit. We accept cash, check, money orders, and VISA, Mastercard and Discover credit cards and debit cards.

Also, please review your insurance coverage to be sure our physicians are members of your plan. You should have been provided a list of member doctors, if your choices are limited by the plan. If you are treated by a physician outside your plan’s network, you may be responsible for a larger portion of the charge, and, with some plans, the entire charge incurred.

If you are a member of an HMO (Health Maintenance Organization), you will need to bring a WRITTEN REFERRAL FORM from your primary care physician to your first visit and all follow-up visits except post-op. Obtaining this form in advance is your responsibility, in order to avoid having to reschedule the appointment. We have tried to assist our patients with this process at their visit when they have not received the form in advance, but many primary care offices are unable to provide these forms the same day.

If you have no insurance, please call before your appointment to discuss the details of a payment plan for any anticipated surgery.
